In this context, how should health systems and providers safely adopt AI to address areas like prior authorization delays while navigating regulatory compliance and patient protections?<\/p>\n<p class=\"wp-block-paragraph\">To gain insights, <em>Digital Journal<\/em> interviewed Jerry Smith, Executive Vice President at D2 Solutions.<\/p>\n<p class=\"wp-block-paragraph\"><strong>Digital Journal: Looking at how AI tools may be used in making prior authorization determinations, what are some potential concerns that come up for you?<\/strong><\/p>\n<p class=\"wp-block-paragraph\"><strong>Jerry Smith:<\/strong> Despite some improvement in Prior Authorization response times by payers, largely driven by legislation, delayed access to medication is still a problem for patients. However, the current prior authorization process continues to be too burdensome for providers and their teams to manage.\u00a0 A process designed to support appropriate care decisions has evolved into one that often creates delays, additional paperwork and unnecessary administrative burden. For a patient waiting to start a medication, those delays can have a meaningful impact on their care.<\/p>\n<p class=\"wp-block-paragraph\">AI has the potential to improve this process by reducing manual work and helping organizations manage complex requirements more efficiently. At the same time, implementation needs to be thoughtful.\u00a0 The biggest concern is ensuring AI works within the realities of healthcare\u2019s existing infrastructure. Many organizations still rely on complex legacy systems, and any technology introduced into these environments must protect patient data, maintain reliability and avoid creating new points of failure.<\/p>\n<p class=\"wp-block-paragraph\"><strong>DJ: Some states have started enacting bills to safeguard patients from the inappropriate use of Artificial Intelligence. What are your thoughts on that?<\/strong><\/p>\n<p class=\"wp-block-paragraph\"><strong>Smith:<\/strong> States introducing AI legislation are recognizing that adoption needs to come with appropriate guardrails and patient protections.\u00a0 The variability of AI\u2019s performance in the healthcare arena in general is significant.\u00a0 Too, as healthcare organizations implement these tools, transparency, accountability and human oversight will be essential to ensuring AI is used responsibly.<\/p>\n<p class=\"wp-block-paragraph\">At the same time, regulations should protect patients while still allowing organizations to use technology to address real challenges. Healthcare organizations should work collaboratively with policymakers to establish reasonable frameworks around areas such as accuracy, transparency requirements, human oversight and bias testing. These safeguards will help build trust while allowing the industry to realize the benefits of AI.<\/p>\n<p class=\"wp-block-paragraph\"><strong>DJ: What other efforts are you aware of to implement guardrails for prior authorization determinations?<\/strong><\/p>\n<p class=\"wp-block-paragraph\"><strong>Smith:<\/strong> Beyond legislation,\u00a0 I am seeing a multi-layered approach that balances automation and efficiency, evidence-based clinical oversight, and not surprisingly, the introduction of published case law into the Prior Authorization equation.\u00a0 This includes the development of clinical logic frameworks, which provide a digital structure between payers and providers. These frameworks can automate administrative tasks while leveraging intelligence to help align the patient\u2019s clinical profile, as documented by the patient\u2019s health care provider, with payer requirements and medical criteria (treatment protocols).<\/p>\n<p class=\"wp-block-paragraph\">By embedding intelligence into the process, organizations can reduce unnecessary friction, limit avoidable denials and help patients access appropriate therapies more efficiently. The goal is not to replace clinical judgment or to rely on relevant legal outcomes, but to create more efficient systems that support providers\u2019 efforts to reach the right therapy for each and every patient by making prior authorization submissions more relevant to the patient in question and easier to navigate.<\/p>\n<p class=\"wp-block-paragraph\"><strong>DJ: Who is responsible for putting up guardrails?<\/strong><\/p>\n<p class=\"wp-block-paragraph\"><strong>Smith:<\/strong> Creating effective guardrails will require collaboration across the healthcare ecosystem. Technology vendors, health systems, pharmaceutical companies, payers and regulators all have a role to play in ensuring AI is implemented in a way that improves processes while maintaining patient safety and trust.<\/p>\n<p class=\"wp-block-paragraph\">There is a meaningful opportunity to improve healthcare workflows through innovation, but adoption must happen responsibly. Providers should not be expected to navigate increasingly complex processes without the right technological and regulatory frameworks in place to support them. And patients need to be kept informed throughout the process<\/p>\n<p class=\"wp-block-paragraph\"><strong>DJ: What other kind of guardrails could be discussed when it comes to AI used in prior authorization decisions?<\/strong><\/p>\n<p class=\"wp-block-paragraph\"><strong>Smith:<\/strong> One of the most important considerations is ensuring organizations have appropriate safeguards around patient privacy, data security and regulatory compliance. Healthcare organizations need confidence that patient information is protected and that AI systems are monitored to ensure they are working as intended.<\/p>\n<p class=\"wp-block-paragraph\">Effective AI implementation should also include ways for patients and providers to question decisions and involve a person when additional review is needed. Transparency will be important as well. Organizations should be able to explain how these systems are being used so stakeholders understand the role AI plays in the process and can trust the outcomes.<\/p>\n<p class=\"wp-block-paragraph\"><strong>DJ: Do you have any advice for the healthcare industry regarding AI tools used in prior authorization decisions?<\/strong><\/p>\n<p class=\"wp-block-paragraph\"><strong>Smith:<\/strong> Healthcare organizations should focus on understanding where AI can provide practical value within existing workflows. Healthcare has historically been cautious in adopting new technologies, and that caution is understandable given the importance of patient safety and data security. At the same time, organizations need to find ways to reduce administrative burden and better support their teams.<\/p>\n<p class=\"wp-block-paragraph\">Rather than replacing existing systems, AI should be integrated thoughtfully into current processes. The greatest opportunity is using these tools to handle routine administrative work so employees can focus on the areas where their expertise matters most.
