Examining the importance of this development and the science behind, in an interview with <em>Digital Journal, <\/em>is\u00a0Dr. Tyler Evans,\u00a0the Founder\/CEO of Wellness Equity Alliance and former Chief Medical Officer for New York City. Beyond influenza itself, how significant is this approval for the future development of vaccines and infectious disease prevention?<\/strong><\/p>\n<p class=\"wp-block-paragraph\"><strong>Tyler Evans:<\/strong> The significance is not that mRNA works. We knew that. It is that the platform cleared a full licensure pathway for a routine seasonal product, reviewed as a standard biologics application, with a 40,805-person randomized trial against an active comparator. Not against placebo. Against the shot people already get.\u00a0 That closes a long-running argument. Critics have said for five years that mRNA was never tested the way ordinary vaccines are tested. Now it has been.<\/p>\n<p class=\"wp-block-paragraph\"><strong>DJ: You have argued that the greatest advantage of mRNA platforms is manufacturing speed rather than superior efficacy. Could you explain how reducing the time between strain selection and vaccine deployment could improve outcomes during both seasonal flu outbreaks and future pandemics?<\/strong><\/p>\n<p class=\"wp-block-paragraph\"><strong>Evans:<\/strong> In a seasonal year, speed buys you a better match. In a pandemic year, speed is the intervention.<\/p>\n<p class=\"wp-block-paragraph\">We choose strains in February for a season that starts in October. That gives the virus roughly eight months to drift away from the vaccine we already committed to. Compress production from six months to two or three and the decision can be made later, with more surveillance in hand. A better-informed guess produces a better-matched vaccine.<\/p>\n<p class=\"wp-block-paragraph\">In a pandemic the arithmetic changes entirely. In 1968 the first batch of vaccine was released 66 days after the production strain became available. That was celebrated as extraordinary speed, and it was. It was also 110,000 doses, most of them for the armed forces, arriving as the wave was already moving through American cities.<\/p>\n<p class=\"wp-block-paragraph\">We have spent nearly sixty years trying to close that gap. This is the first real compression.<\/p>\n<p class=\"wp-block-paragraph\"><strong>DJ: Influenza viruses are notorious for antigenic drift and strain mismatch. How much of a game changer is mRNA technology in addressing this long-standing challenge compared with traditional egg-based vaccine production?<\/strong><\/p>\n<p class=\"wp-block-paragraph\"><strong>Evans:<\/strong> I\u2019d call mRNA technology a genuine game changer, mainly because of speed and precision. Traditional egg-based production locks us into strain selection roughly six months ahead of flu season, and the adaptation process\u2014getting a strain to grow well in eggs\u2014can actually introduce mutations that shift its antigenic profile, so we\u2019re not always matching what we intended to match in the first place.<\/p>\n<p class=\"wp-block-paragraph\">mRNA sidesteps that. Once you have the genetic sequence, you can manufacture a vaccine in weeks, not months, without the egg-adaptation step. That means we can select strains closer to flu season using more current data, and get a tighter match with what\u2019s actually circulating. It also gives us more flexibility to update formulations quickly if a major drift event happens.<\/p>\n<p class=\"wp-block-paragraph\">What it doesn\u2019t do is solve antigenic drift itself\u2014the virus still mutates, and strain selection is still a forecasting exercise. But it dramatically improves our ability to respond quickly and accurately once we\u2019ve made that call. I\u2019d say it\u2019s one of the most meaningful advances in flu vaccine production in decades\u2014an evolution in how we respond to the problem, not a cure for the problem itself<\/p>\n<p class=\"wp-block-paragraph\"><strong>DJ:<\/strong> <strong>What lessons from the COVID-19 response should policymakers apply now to ensure mRNA manufacturing capacity is available when the next novel pathogen emerges?<\/strong><\/p>\n<p class=\"wp-block-paragraph\"><strong>Evans:<\/strong> A key lesson we learned is that the last mile fails first. In 2021 we had doses before we had a way to get them to the people at highest risk. Manufacturing capacity without distribution capacity produces vaccines in warehouses.<\/p>\n<p class=\"wp-block-paragraph\">Another key lesson is that capacity decisions made now determine what exists later, and later is not a date we get to choose.Manufacturing capacity is not something you build during an emergency. It is something you maintain between them.A manufacturing\u00a0 line that produces seasonal vaccines every year can be redirected. A line that has been shut down takes eighteen months to restart, which is longer than most pandemic waves.<\/p>\n<p class=\"wp-block-paragraph\"><strong>DJ: What practical steps should healthcare systems take to prevent mFlusiva from becoming a vaccine primarily available to those with the financial means and resources to obtain it?<\/strong><\/p>\n<p class=\"wp-block-paragraph\"><strong>Evans:<\/strong> Three things need to happen right now, not in October.<\/p>\n<p class=\"wp-block-paragraph\">First, know your baseline. The court order reverted us to the pre-January 5, 2026 immunization schedule\u2014it didn\u2019t create a policy vacuum. Any protocol built on the January revisions needs to be pulled back immediately. Confirm coverage in writing with every payer: Medicare Part B covers flu vaccine for patients 65 and older by statute, but commercial coverage for adults 50 to 64 is currently voluntary. Brief clinicians on that gap before September, and decide now how you\u2019ll handle uninsured patients at the counter. Don\u2019t wait on the litigation\u2014the appeal moves on its own timeline, but flu season doesn\u2019t.<\/p>\n<p class=\"wp-block-paragraph\">Second, autumn purchasing decisions are being finalized this month. Systems serving low-income populations need a seat at that table now, not later.<\/p>\n<p class=\"wp-block-paragraph\">Third, put doses where people already are\u2014churches, libraries, transit hubs, shelters, encampments\u2014not just pharmacies and clinics. Staff those sites with community health workers who have lived experience. And fix the access barrier we built ourselves: Medicare only pays for community health integration services after an initiating visit with a billing practitioner. People living outside don\u2019t have that visit. We created an outreach payment code, then locked it behind a clinic appointment. Provided by Dr. Tyler Evans, with permission. <\/figcaption><\/figure>\n<p class=\"wp-block-paragraph\"><strong>DJ: What evidence have you seen that community-based healthcare delivery can overcome vaccine hesitancy and improve uptake among underserved populations?<\/strong><\/p>\n<p class=\"wp-block-paragraph\"><strong>Evans:<\/strong> Most of what gets called hesitancy is inaccessibility. A person who cannot take four hours off work is not hesitant.<\/p>\n<p class=\"wp-block-paragraph\">Fix proximity first, and you learn how much genuine hesitancy remains. It is consistently less than assumed.\u00a0 The evidence is stronger than people realize.<\/p>\n<p class=\"wp-block-paragraph\">In Marin County, in the first two months of COVID-19 vaccine availability, my team analyzed our own data. More than 90 percent of the people we had vaccinated were white, with personal transport and digital access. We called it the triple Cs: Caucasians with cars and computers. We were reaching the people who could already reach us.<\/p>\n<p class=\"wp-block-paragraph\">A large health system in Miami partnered with churches, synagogues, and mosques across Miami-Dade County to vaccinate people 65 and older, and doubled the local vaccination rate among Black older adults within a week.\u00a0<\/p>\n<p class=\"wp-block-paragraph\">Emory University\u2019s Interfaith Health Program built a framework with CDC and local health departments that raised influenza vaccine uptake before it was ever applied to COVID-19.<\/p>\n<p class=\"wp-block-paragraph\"><strong>DJ:<\/strong> <strong>What impact do mixed policy signals have on public confidence in science, and how can trust be rebuilt?<\/strong><\/p>\n<p class=\"wp-block-paragraph\"><strong>Evans:<\/strong> The public is better at detecting inconsistency than we give them credit for. They may not follow the regulatory detail, but they notice when a government defunds a technology and then approves it. What they take from that is not a nuanced view of institutional independence. What they take is that someone is not telling them the whole story.<\/p>\n<p class=\"wp-block-paragraph\">The damage compounds because confidence was already fragile, and it was fragile for reasons that predate any of this. Communities that were experimented on, neglected, or lied to did not become skeptical last year.\u00a0 Rebuilding is slower and less satisfying than people want. It is not a messaging campaign. It is consistency over time. In every setting I have worked, from South Sudan to South Los Angeles, the finding is the same. Trust is a delivery problem before it is a communications problem.<\/p>\n<p class=\"wp-block-paragraph\"><strong>DJ:<\/strong> <strong>For older adults considering vaccination this season, and for healthcare professionals advising them, what key message would you like to emphasise about influenza risk, vaccine choice, and the importance of timely vaccination?<\/strong><\/p>\n<p class=\"wp-block-paragraph\"><strong>Evans:<\/strong> Three things:<\/p>\n<ol class=\"wp-block-list\">\n<li>Influenza kills tens of thousands of Americans a year, and adults over 65 carry most of that burden. That has been true for decades and it is true this year.<\/li>\n<li>The best flu vaccine is the one you can actually get, and getting it early matters more than which one you get. If you are 68 and doing well on a high-dose vaccine, there is no evidence yet that you should switch. We do not have head-to-head data against high-dose. That study is running now and reports in a couple of years.<\/li>\n<li>Do not wait for the perfect vaccine. Every season I meet people who postponed the decision until it was made for them.
