ISO is cheap and many times more powerful than fentanyl \u2013 making for a potential crisis.<\/p>\n<figure class=\"nyp-slideshow-modal-image wp-block-image aligncenter size-large\"><figcaption>A viral video captured users under the effects of ISO in San Francisco\u2019s Tenderloin district. <\/figcaption><\/figure>\n<figure class=\"nyp-slideshow-modal-image wp-block-image aligncenter size-large\"><figcaption>ISO users often stoop over or collapse. <\/figcaption><\/figure>\n<p>\u201cEven a single pill can contain a lethal dose,\u201d the officials said in their alert, which noted that the two synthetic opioids would not show up in tests that indicate the presence of fentanyl, making them even more dangerous because they\u2019re undetectable. \u00a0<\/p>\n<aside class=\"single__inline-module aligncenter wp-block-nypost-editor-primary-tag\">\n<\/aside>\n<p>A shocking video posted online showed throngs of stupefied users reeling from the effects of the deadly drug in the city\u2019s notorious Tenderloin district.<\/p>\n<figure class=\"wp-block-embed is-type-rich is-provider-x wp-block-embed-x\">\n<div class=\"wp-block-embed__wrapper\">\n<div class=\"embed-wrapper twitter\">\n<blockquote class=\"twitter-tweet\" data-width=\"550\" data-dnt=\"true\">\n<p lang=\"en\" dir=\"ltr\">This is the scene in San Francisco&#8217;s Tenderloin neighborhood. Rufo \u2694\ufe0f (@christopherrufo) June 4, 2026<\/p><\/blockquote>\n<\/div>\n<\/div>\n<\/figure>\n<p>In the video created by Manhattan Institute writer Christopher Rufo, addicts are seen stooping over from the effects of the powerful drug and even lying corpselike on the sidewalk.<\/p>\n<p>Rufo said a dealer told him ISO is becoming more popular on the streets.<\/p>\n<p>\u201cFor the last ten years, it\u2019s been an arms race for increasing the strength of opioids, from heroin to fentanyl, and now, to ISO,\u201d said Rufo. \u201cIt\u2019s devastating California cities.\u201d<\/p>\n<figure class=\"nyp-slideshow-modal-image wp-block-image aligncenter size-large\"><figcaption>ISO users appeared corpselike on the street. <\/figcaption><\/figure>\n<figure class=\"nyp-slideshow-modal-image wp-block-image aligncenter size-large\"><figcaption>Users may not know they\u2019re consuming the deadly drug. <\/figcaption><\/figure>\n<p>According to the Center for Forensic Science Research and Education, isotonitazene, which is also known as Nitazine, is roughly 20 times more potent than fentanyl. It may be smoked, used intravenously or taken in a pill. \u00a0<\/p>\n<p>Officials with the federal Drug Enforcement Administration last month issued a new warning about ISO and other synthetic drugs that the DEA says may be mixed with fentanyl.<\/p>\n<p>\u201cThese emerging synthetic drugs can be significantly more powerful than fentanyl and greatly increase the risk of suffering a fatal overdose,\u201d the advisory warned, noting that users who OD on the drugs may not be responsive to the overdose-reversal drug naloxone.<\/p>\n<p>According to the DEA, ISO initially emerged in U.S. illicit drug markets in 2019, when it was first identified in seizures of illegal drugs by law enforcement. ISO is not approved for medical use and has no industrial use.\u00a0<\/p>\n<figure class=\"nyp-slideshow-modal-image wp-block-image aligncenter size-large\"><figcaption>The drug may be taken intravenously,\u00a0smoked or ingested in a pill.   <span class=\"credit\">AP<\/span><\/figcaption><\/figure>\n<figure class=\"nyp-slideshow-modal-image wp-block-image aligncenter size-large\"><figcaption>Dealers said ISO is gaining popularity in San Francisco.  <span class=\"credit\">Roman Savi<\/span><\/figcaption><\/figure>\n<p>ISO is a \u201cserious concern to the public safety,\u201d the DEA said in another bulletin issued on the drug in December 2025. \u00a0\u00a0<\/p>\n<p>The DEA has counted more than 900 reports of isotonitazene since it was initially identified by the agency in 2019, including 42 records in 2019, a peak of 358 reports in 2021, and 43 reports in 2024, the most recent year for which the data was available.<\/p>\n<p>Law enforcement officials in Los Angeles said ISO has not yet become a threat in California\u2019s biggest city, but one police source said that \u201cbathtub pharmacists\u201d there are increasingly mixing fentanyl with a variety of other drugs, including animal tranquilizers.<\/p>\n<p>Los Angeles health officials blame fentanyl for just over half of all drug and alcohol deaths in the city. Police have recently mounted a push to cleanse MacArthur Park in downtown LA of the crowds of users who purchase and consume drugs there.<\/p>\n<figure class=\"nyp-slideshow-modal-image wp-block-image aligncenter size-large\"><figcaption>Anti-drug activist Jacqui Berlinn said ISO is \u201cincredibly scary.\u201d   <span class=\"credit\">Pete Thompson for NYPost<\/span><\/figcaption><\/figure>\n<p>Jacqui Berlinn, who cofounded the San Francisco-based activist group Mothers Against Drug Addiction and Deaths, said San Francisco police ought to be doing more to clean up the streets of that city.<\/p>\n<p>Berlinn said the rise of ISO is particularly threatening because the drug is so powerful and it can\u2019t be detected by fentanyl test kits.<\/p>\n<p>\u201cIt\u2019s incredibly scary because it\u2019s taking the addicts unaware,\u201d said Berlinn.<\/p>\n<\/p><\/div>\n<p><script async src=\"\/\/platform.twitter.com\/widgets.js\" charset=\"utf-8\"><\/script><br \/>\n<br \/><!-- \u603b\u5bb9\u5668\uff1a\u6700\u5927\u5bbd908px Grid\u7d27\u51d1\u5e03\u5c40 -->\r\n<div style=\"display: grid;
